I tape this survey to each clipboard and encourage evangelists to use the questions listed to get into Gospel conversations. You can go through the entire survey with people, but I oftentimes just pick 1 or 2 of the questions to ask. I also encourage evangelists to come up with their own questions and to listen to the leading of the Holy Spirit.

Best if printed on 100-lb white or cream card stock. The borders are navy blue, but black and white printing is just fine for this certificate.
Praise God for those moments when a person confesses Jesus as their Savior! You can give them one of these and walk them through what exactly it means to be a Christian. You can use these in church and when evangelizing outside of the church. You can make an entire kids or youth lesson out of going over the certificate. Someone might get saved just by learning about what it means to be a Christian. The truths of God are mighty to save!

A classic prayer a person prays after receiving Jesus. The sinner’s prayer does not save a person, but it can be used to guide them through their first prayer as a child of God.

You can use this form at evangelism events or when street evangelizing if you want to collect contact information for follow-up. I usually ask for contact information if a person seems interested in coming to church with me, makes a profession of faith, or is interested in speaking more. I do not use prayer request boxes because I like to encourage people to engage in conversation, and sometimes people leave a request in a box without filling in their contact information. The contact intake form makes it easy to see whether or not a person has left their contact information.
